If your child requires routine or as needed medications to be administered during the week of camp, the form above must be completed by your child’s Physician/Health Care Provider. Physician/Health Care Provider Signature or Office Stamp are REQUIRED.
It is time to start planning Camp Pollock's Summer Youth Camps. Please help us in promoting and spreading the word about the upcoming camping program. Young lives are being transformed by God through the ministry of Camp Pollock. Attached to this letter you will find a camp registration form, a camp medical form, waiver and liability forms, and camper/staff guidelines. Please note that the grade designations for this year's youth camps are the same as 2025. The grades listed are meant to indicate the grade that the student will have completed by the end of the 2025-26 school year. There are three weeks of camps:
